At SYNERGY COSMETIC SURGERY AND MED SPA, LLC, we are committed to providing professional, safe, and personalized cosmetic surgery and medical spa services.
These policies help us maintain appropriate scheduling, patient safety, privacy, and financial practices.
These policies do not replace procedure-specific informed-consent documents, surgical agreements, financial agreements, the Notice of Privacy Practices, or other documents that may apply to your care.
1. Medical Care
Medical services are provided by Steven Chang, MD, and/or other appropriately licensed health care professionals acting within their legally permitted scopes of practice.
The practitioner providing a particular service will depend on the service, the practitioner’s qualifications, and the patient’s individual medical needs.
All treatments are subject to medical evaluation and professional clinical judgment.
The practice may decline, postpone, modify, or discontinue a treatment when the treating practitioner determines that doing so is medically appropriate or necessary for patient safety.
2. Cosmetic Surgery
Cosmetic surgery is elective medical care.
A consultation does not guarantee that a patient will be considered an appropriate candidate for a particular procedure.
Before surgery, patients may be required to complete medical evaluations, provide medical history and medication information, obtain medical clearance, complete laboratory or diagnostic testing, or satisfy other pre-operative requirements.
The physician will discuss the procedure, anticipated benefits, material risks, alternatives, recovery, and other relevant considerations as appropriate.
Separate informed-consent and surgical financial documents may apply.
3. Medical Spa Services
Medical spa services may include injectable treatments, aesthetic procedures, skin treatments, body treatments, medically supervised therapies, and other medical or aesthetic services.
Treatment eligibility is determined on an individual basis.
Certain treatments may require a medical evaluation before treatment and may not be appropriate for all patients.
4. No Insurance Accepted
SYNERGY COSMETIC SURGERY AND MED SPA, LLC is a cash-pay practice and does not accept or bill health insurance.
Patients are responsible for payment of all services provided by the practice.
The practice does not submit insurance claims on behalf of patients.
If appropriate documentation is available for a patient to submit independently to an insurance company, the practice may provide such documentation upon request.
The practice does not guarantee that an insurance company will reimburse a patient for any service.
5. Consultations
Consultations provide an opportunity to discuss the patient’s goals, medical history, treatment options, expected results, risks, and alternatives.
A consultation does not guarantee that treatment will be recommended or performed.
The physician may recommend against a requested treatment when it is not medically appropriate or when the anticipated risks outweigh the potential benefits.
6. Appointment Scheduling
Appointments reserve dedicated provider time and practice resources.
A valid credit or debit card may be required to reserve an appointment.
Patients may be required to complete registration and medical forms before their appointment.
7. Appointment Confirmation
Appointments are confirmed when scheduled.
The practice may send appointment reminders by telephone, text message, or email.
Reminders are a courtesy and do not relieve patients of responsibility for attending or cancelling appointments.
8. Arrival Time
Patients should arrive approximately 15 minutes before their scheduled appointment.
Patients who arrive late may have less time available for their appointment.
If the appointment cannot be safely completed within the remaining time, it may need to be rescheduled.
9. Cancellations and No-Shows
Because appointments reserve provider time and practice resources:
- Cancellations with less than 24 hours’ notice may incur a $75 fee.
- Same-day cancellations may incur a $100 fee.
- No-shows may incur a $100 fee.
The practice may waive a fee when circumstances warrant, including documented emergencies.
These fees are administrative charges associated with reserved appointment time and are not charges for medical treatment.
10. Surgical Scheduling and Deposits
Cosmetic surgery requires substantial scheduling and preparation.
A surgical procedure may require an advance deposit or payment to reserve a surgical date.
The amount and terms of any surgical deposit will be disclosed before payment.
Because surgery may involve operating facilities, anesthesia providers, surgical staff, equipment, and other resources, additional cancellation and rescheduling terms may apply to surgical procedures.
Those terms will be provided in the applicable surgical financial agreement.
11. Pre-Procedure Requirements
Patients may be required to complete certain requirements before treatment or surgery, including:
- Medical history
- Medication and supplement information
- Allergy information
- Laboratory testing
- Medical clearance
- Pre-operative examination
- Medication instructions
- Fasting instructions
- Transportation arrangements
- Post-operative assistance arrangements
- Other procedure-specific requirements
Failure to complete required preparation may result in cancellation or rescheduling of the procedure.
Applicable cancellation or rescheduling fees may apply.
12. Post-Procedure Care
Patients are expected to follow all post-treatment and post-operative instructions provided by the practice.
Patients should promptly contact the practice regarding unexpected symptoms or concerns following treatment.
For a medical emergency, call 911 or seek immediate emergency medical care.
13. Payment
Payment is due when services are rendered unless otherwise specified in a written agreement.
We accept major credit cards, debit cards, and cash.
We do not accept health insurance.
Certain procedures, products, treatment packages, and surgical services may require payment in advance.
14. Financing
Third-party financing may be available for certain services.
Financing approval and financing terms are determined by the financing company and are separate from the practice’s medical services.
The practice does not guarantee financing approval.
15. Outstanding Balances
Patients are responsible for payment of all charges for services and products received.
Outstanding balances may need to be resolved before additional elective services are provided, subject to applicable law and the patient’s medical circumstances.
16. Refunds and Prepaid Services
Refund and cancellation terms for prepaid services, treatment packages, memberships, surgical deposits, and other purchases will be disclosed at the time of purchase.
Services that have already been provided are generally not refundable.
Any refund policy is subject to applicable Florida and federal law.
17. Gift Cards and Gift Certificates
Purchased gift cards and gift certificates are subject to applicable Florida law.
Purchased gift certificates may not be assigned an expiration date or post-sale fee where prohibited by Florida law.
Promotional credits or complimentary certificates provided without a separate purchase may be subject to their disclosed promotional terms to the extent permitted by law. Florida law expressly distinguishes purchased gift certificates from certain promotional certificates. (Florida Senate)
18. Medical Information
Patients are responsible for providing complete and accurate information regarding:
- Medical history
- Current medications
- Allergies
- Prior surgeries
- Previous cosmetic procedures
- Medical conditions
- Pregnancy or possible pregnancy
- Breastfeeding
- Relevant lifestyle factors
- Other information requested by the practice
Failure to provide accurate information may affect treatment safety and results.
19. Pregnancy
Patients should notify the practice if they are pregnant, may be pregnant, or are breastfeeding.
Certain medications, injectable treatments, aesthetic procedures, and surgical procedures may need to be postponed or avoided.
20. Medications
Patients should provide a complete and accurate list of prescription medications, over-the-counter medications, vitamins, supplements, and other substances they use.
Patients should not discontinue prescribed medications unless instructed to do so by the prescribing physician or another qualified health care professional.
21. Medical Weight Management
Where medical weight-management services are offered, medications and treatment plans are prescribed only following appropriate medical evaluation and when determined to be appropriate by the treating practitioner.
Patients must disclose relevant medical conditions, medications, allergies, and other information necessary for safe treatment.
22. Injectable Treatments
Injectable treatments may include neuromodulators, dermal fillers, biostimulatory injectables, platelet-based treatments, and other medical aesthetic treatments.
The specific treatment, product, dosage, injection sites, and treatment plan are determined based on individual patient needs and clinical judgment.
No particular result is guaranteed.
23. Results
Individual results vary.
No cosmetic, aesthetic, surgical, or medical result can be guaranteed.
Patients should have realistic expectations regarding the potential benefits, limitations, risks, recovery, and outcomes associated with their treatment.
24. Before-and-After Photographs
Before-and-after photographs may be used to demonstrate examples of treatment results.
Individual results vary.
Photographs do not guarantee that another patient will achieve the same or similar result.
Patient photographs used for marketing purposes will be obtained and used in accordance with applicable law and required authorizations.
25. Patient Rights
Patients have the right to:
- Receive respectful and considerate care.
- Receive information about proposed treatments.
- Ask questions about recommended procedures.
- Participate in decisions regarding their care.
- Decline treatment, subject to applicable law.
- Receive information regarding financial responsibility.
- Request access to medical records as permitted by law.
- Have health information handled in accordance with applicable privacy laws.
- Raise concerns about their care.
26. Patient Responsibilities
Patients are expected to:
- Provide accurate and complete medical information.
- Follow pre-treatment and post-treatment instructions.
- Follow pre-operative and post-operative instructions.
- Inform the practice of relevant changes in health or medications.
- Arrive on time.
- Provide reasonable notice when cancelling or rescheduling.
- Treat physicians, practitioners, employees, and other patients respectfully.
- Meet financial obligations for services received.
27. Medical Records
The practice maintains medical records in accordance with applicable federal and Florida law.
Patients may request access to or copies of their medical records in accordance with applicable law and the practice’s records-request procedures.
Applicable fees permitted by law may apply to certain records requests.
28. Privacy
Patient health information is maintained and disclosed in accordance with applicable federal and Florida privacy laws, including HIPAA where applicable.
The practice’s Privacy Policy provides additional information about how protected health information may be used and disclosed and about patient privacy rights.
29. Electronic Communications
The practice may use telephone, text messaging, email, patient portals, and other electronic communication methods.
Patients should use secure communication methods provided by the practice when transmitting sensitive medical information.
Electronic communications should not be relied upon for emergency medical care.
30. Text Messaging
Patients who provide consent for applicable messaging programs may receive text messages concerning appointments, administrative matters, treatment-related communications, or promotional offers.
Message frequency varies.
Message and data rates may apply.
Promotional messages may be discontinued by replying STOP where supported.
Administrative communications necessary to manage an appointment or patient relationship may continue where legally permitted.
31. Children and Minors
Certain cosmetic procedures and medical treatments may have minimum age requirements.
Patients under 18 may require parental or legal-guardian consent or participation when required by law.
Children who are not patients should not accompany patients into treatment or procedure areas unless specifically approved by the practice.
32. Service Animals and Pets
Service animals will be accommodated as required by applicable law.
Pets and animals that do not qualify as service animals are generally not permitted in patient-care or procedure areas.
33. Emergencies
SYNERGY COSMETIC SURGERY AND MED SPA, LLC is not an emergency department and does not provide 24-hour emergency medical services.
If you are experiencing a medical emergency, call 911 or proceed to the nearest emergency department.
Do not rely on voicemail, email, text messaging, or online forms for emergency medical concerns.
34. Referrals
We appreciate patients who recommend our practice to friends and family.
Any patient referral or promotional program offered by the practice will be structured and administered in accordance with applicable Florida and federal law.
35. Promotional Offers
Promotional offers are subject to their stated terms and eligibility requirements.
Certain promotions may be limited to new patients, specific treatments, specified dates, or other eligibility criteria.
Where Florida law requires a disclosure for advertisements involving free, discounted-fee, or reduced-fee medical services, the applicable disclosure will be included with the advertisement. Florida Statutes §456.062 requires specific capitalized language in qualifying advertisements. (Online Sunshine)
36. Office Conduct
Patients, visitors, and staff are expected to maintain a respectful and professional environment.
The practice does not tolerate threats, harassment, abusive behavior, violence, discrimination, or inappropriate conduct toward physicians, practitioners, employees, patients, or visitors.
The practice may take appropriate action when conduct creates a safety concern or materially interferes with patient care or office operations.
37. Changes to Office Policies
The practice may modify these policies from time to time.
Patients will be informed of material policies applicable to their appointments, purchases, or procedures as appropriate.
Nothing in these policies is intended to waive or restrict rights or protections that cannot legally be waived.
38. Governing Law
These policies are governed by the laws of the State of Florida, except to the extent applicable federal law controls.
Nothing in these policies is intended to waive or eliminate any right or protection that cannot legally be waived.
